Care at Home

At St Michaels we have been providing end of life or palliative care for many years, which means we fully understand how special this stage of life for a person and their loved ones.

We know it can be emotionally overwhelming for you and your family and that you may have many questions and concerns you feel are important and need to be respected.  You will have up days and down days, and our care is always sensitively provided with this in mind.

Our aim is to provide care that helps and is built around your needs. We are there to ensure you feel supported and cared for and to take any pressure off your mind. Our care can help you find relief from pain, discomfort and anxiety, because knowing someone is there to take care of the little things that make life easier can make a big difference. We also understand that for many people, being able to spend their final days at home, surrounded by family and things that are familiar, is hugely important to them.

At the centre of our care is your dignity so we will always respect your preferences and personal requests. Our carers are fully trained to offer end of life care and always start by listening to you to fully understand what you need and would like. They understand that things can change from day to day and keep this in mind. They will always do their utmost to ensure every request is fully met and to discuss what is happening with you. If you would like, we can also discuss your care needs with the key people in your life, such as family and friends, to help support you with the care you need throughout this time. Key to this time of life for many people is maintaining connection with the things that represent your beliefs or faith, if you have one, and our carers will respect and keep that in mind too.

End of Life Care at Home is designed to be person-centred and to help make you as comfortable as we can. That may mean:

  • Helping you to get washed and dressed in the mornings.
  • Helping you to get ready for bed during the evenings.
  • Helping you to take your medication or to prepare and eat meals.
  • Making sure you are comfortably positioned and turned in bed or, if you feel up to it, to take a break from bed and sit outdoors.
  • Helping you to get out and about if you have errands to complete or want to visit special places or friends and family.
  • We can work with other individuals and service providers involved in your care, such as hospices, hospitals, physiotherapists and health professionals.
  • We can help deliver peace of mind by ensuring you have important documents or arrangements in place.

Everyone is different and so the end of life support and care they need will be different, and this is why every care package begins with understanding your needs.
